Position Information

Title: Juvenile Rehabilitation Case Manager

FLSA Status: Hourly, Full-Time, Non-Exempt

Location: Yakima County

Reports to: Outreach Program Manager

Overview (what unique role does this position play within Rod’s House?)

Case Managers at Rod’s House focus on exiting a young person out of homelessness and into safe and stable housing through emotional, employment, educational, and housing supports. Approximately 25% of young people who are released from juvenile rehabilitation facilities in our state, either exit to homelessness or are homeless within one year of release. The Juvenile Rehabilitation Case Manager works with vulnerable young people (ages 13-24) at risk of homelessness or housing instability being released from juvenile rehabilitation facilities. The Juvenile Rehabilitation Case Manager plays a pivotal role in prioritizing the accessibility of services and meeting the unique needs of youth and young adults exiting the criminal justice system without a safe place to go.

Duties and Responsibilities

Direct Case Management

  • Establish positive relationships with incarcerated youth (ages 13-17) and young adults (ages 18-24):
  • Conduct Rod's House intake interviews for incarcerated youth needing housing;
  • Collaborate with juvenile rehabilitation staff to develop individual case plans incorporating young people's responsibilities;
  • Connect with the Department of Children, Youth and Families to identify young people at risk of homelessness or housing instability upon release from Juvenile Rehabilitation;
  • Maintain consistent in-person meetings, providing support and services related to housing plan goals, focused on immediate referral and enrollment in a shelter or housing option upon exit;
  • Participate in Review Team meetings for youth that will be released;
  • Provide aftercare to formerly incarcerated youth in order to smoothly transition services to their new Case Manager;
  • Provide assistance navigating the justice, health care and education systems;
  • Serve as a connection to a wide range of community-based services, including employment and education opportunities within the community;
  • Facilitate recreational, vocational, and life/job skill-training activities for young people on caseload;
  • Complete Coordinated Entry housing assessments for each young person to ensure they are active in HMIS;
  • Maintain accurate and quality client documentation in HMIS and other databases Rod’s House uses;
  • Collaborate with Outreach Team staff to support the needs of incarcerated youth including transports, distributing basic needs, and other support services;
  • Provide weekly updates on active caseload during supervision meetings with Outreach Program Manager.
  • Maintain clear, professional, and timely participant contact notes, service plans, progress updates, statistical records, and other documentation according to all confidentiality laws and guidelines;

Other Outreach Responsibilities

  • Coordinate with other case management staff across all Rod’s House sites;
  • Collaborate with other Outreach staff to represent Rod’s House at community events;
  • Build relationships with community agencies and other Case Managers to maximize referral resources to assist young people in enrolling into safe and stable housing;
  • Participate in local and state interagency work groups, as needed;
  • Participate in the annual Homeless Point In Time count;
  • Support other Outreach operations, as needed.

Other Responsibilities

  • Establish positive relationships and provide trauma-informed and culturally appropriate care;
  • Submit End of Month reports to supervisor;
  • Participate in regular supervision meetings with supervisor and attend program, department, and agency meetings and trainings;
  • Perform regular safety inspections of required work materials and vehicles.

Complete all duties as assigned and other duties as required.

Requ ired or Preferred Education and/or Experience

  • One of the following ( required ):
    • A master’s degree in social services or a closely related field from an accredited school and one year of experience working with youth or young adults OR;
    • A bachelor’s degree in social services or a closely related field from an accredited school and tw0 years of experience working with children or youth OR;
    • A high school diploma or equivalent and five years of successful full-time experience in a relevant field working with children or youth; and (i) supervisory abilities that promote effective staff performance; and (ii) relevant experience, training, and demonstrated skills in each area that they will be managing or supervising.
  • An understanding of and adherence to a strengths-based philosophy with youth and young adults ( required );
  • Dedication to promoting an inclusive and diverse workplace through competency in equity, race, social justice, differently-abled, and LGBTQIA2S+ identities ( required );
  • Bilingual (Spanish-English) ( strongly preferred );
  • Previous experience working with the juvenile justice and juvenile rehabilitation systems and their resources ( strongly preferred );
  • Experience working with young people who have experienced homelessness, sexual exploitation, child abuse/neglect, domestic violence, substance misuse, mental health symptoms, and family dynamics ( strongly preferred );
  • Experience networking and advocating with other agencies on behalf of young people to access housing, medical, financial, legal, education and other resources ( strongly preferred );
  • Knowledge in human services, social work, education, or a related field ( preferred );
  • Lived experience relevant to the population served ( preferred ).

  • Must be at least 18 years of age ( required );
  • Pass a Washington State criminal background check ( required );
  • Pass a Federal (FBI) criminal background check ( required );
  • Valid driver’s license and auto insurance is ( required );
  • Possess skills and ability to work successfully with the unique needs of children in care;

Work Conditions and Physical Requirements

This position requires daily interactions with youth experiencing homelessness, community members, and other social service providers. Physical requirements for the position include frequent walking, hearing, seeing, speaking, stooping, kneeling, crouching, reaching, grasping, climbing, and repetitive motions. This position could include exerting up to 50 pounds of force to lift, carry, pull, or move objects.

Salary/Compensation and Benefits

  • Starting hourly wage of $18.50 per hour;
  • 40 hours per week, Monday-Friday;
  • Within 30 days, employer pays medical (90%), dental (90%), and vision (90%) insurance;
  • 10 paid Washington state holidays;
  • 15 days of paid time off granted immediately after 90 days, increasing by 5 days every 2 years;
  • 2 hours of paid sick leave accrued for every 40 hours worked, with up to 80 hours eligible for annual rollover;
